New Client Paperwork
2. Click yes to proceed
3. Click on Patient Login/Profile (left side of screen)
4. Answer “yes” that you are a new user
5. Complete questions & click “Save My Information” at bottom
6. Go to personal email address to retrieve temporary password
7. Go back to www.naicforms.org & sign in using temporary password.
8. Read information about e-signatures & choose to “Accept” or “Do not Accept”
9. Click on Profile (top of page) to reset password; click “Save My Information”
10. Click on “Patient Forms”
11. Complete the following forms (6): Consumer ID, How to be Contacted, Intake Process for Fee Agreements, Fee Agreement Contract, Fee Schedule Computation, Consumer Handbook.
12. As each form is completed click “Send to NAIC”. If you need to stop & go back to the form click “Save Progress”. Once either option is chosen, click on “Patient Forms” to finish each.
13. All forms are forwarded to a secure site at NAIC. Upon completion you may come to NAIC at anytime, M-F, 8-3, to begin the walk-in assessment process. Assessments are approximately 1-2 hours and are completed on a first come – first serve basis.
